- Pinned/crushed by a falling satellite dish antenna
- Crushed in his jeep when a gravity well forms on the spot
- Electrocuted when he uses his power to supplement the power supply for the device that can stop the formation of the gravity wells

- S 3, Ep 9, "Welcome Back, Carter" > Jack Carter's temporary replacement after Carter was fired for letting Eva Thorne escape. S.A.R.A.H. tried to "kill" Sheriff Andy several times so that Carter wouldn't have to leave, creating out-of-control gravity wells that almost destroyed Eureka.
